The short answer
Physiotherapy payment in B.C. can involve different routes: a private extended health plan, a public program with its own eligibility rules, a claim administrator, or self-payment. Check the route that applies to you before treatment begins.
Do not treat a clinic’s statement that it “direct bills” as confirmation that your plan will pay the full amount.
Does MSP cover physiotherapy?
Sometimes, but only for people who meet the Province’s eligibility rules for MSP Supplementary Benefits. The current B.C. source says eligible beneficiaries can receive a partial contribution for physical therapy from approved providers, subject to the program’s combined annual visit limit. It is not the same as private extended health benefits, and it does not make every private physiotherapy visit fully covered.
Private extended health coverage is separate. Your insurer or benefits administrator decides whether your plan recognizes the practitioner, what annual amount remains, and whether there is a deductible, co-insurance or per-visit cap.
Hypothetical example — not a quote or coverage promise: If an appointment costs $120 and an eligible payer contribution is $23, the remaining amount could be $97 before considering any other applicable benefit. Your actual cost depends on eligibility, provider billing and your plan.
Read the Government of B.C. Supplementary Benefits page before relying on a contribution.
Separate the coverage questions
| Question | Why it matters |
|---|---|
| What exact appointment is being billed? | Initial assessments, follow-ups and add-ons may not be treated the same way |
| What is the practitioner’s designation? | Benefit plans can set eligibility by designation |
| What remains in my benefit year? | An annual maximum may be partly or fully used |
| Is there co-insurance or a per-visit cap? | You may owe part of the approved amount |
| Is prior approval needed? | Some plans or claims require it before treatment |
| Can the clinic direct bill my plan? | This affects the payment process, not the underlying eligibility |
Before your first appointment
- Call or use the payer’s secure channel. Identify the exact service as physiotherapy and give the practitioner designation if known.
- Ask the clinic for the initial appointment fee, follow-up fee, and any separately billed items before you book.
- Confirm whether a physician referral or a treatment plan is needed for your payer—not only for the clinic.
- Ask whether the clinic can direct bill your specific plan. If not, ask for an itemized receipt and proof of payment.
- Keep the payer’s reference number or written answer with your own records.
Direct billing: practical limits
A clinic may offer direct billing for certain insurers or plans. The payer can still reduce or decline part of a claim based on eligibility, plan limits, missing information, or the service date. Ask the clinic what it submits and what balance you would be responsible for if the claim is not fully paid.
If you are using a claim program
Auto or workplace injury claims can have separate authorization, provider and invoicing rules. Confirm those rules with the claim administrator before relying on coverage. This guide does not determine claim eligibility.
A concise call script
“I am considering an initial physiotherapy appointment in B.C. Can you confirm whether physiotherapy with this practitioner designation is eligible under my plan, what balance remains, whether approval is needed, and whether I should use direct billing or submit a receipt?”
Frequently asked questions
Is physiotherapy covered by MSP in B.C.?
Only eligible people can receive the Province’s partial Supplementary Benefits contribution for approved physical therapy. Private physiotherapy visits are not automatically fully paid by MSP. Confirm your current eligibility with the official B.C. source.
Will my extended health plan cover my physiotherapist?
It depends on your plan and the practitioner’s designation. Confirm the exact provider eligibility, remaining benefit and any per-visit restriction with the payer.
Does direct billing remove all out-of-pocket cost?
No. A deductible, co-insurance, used annual maximum, non-covered fee or balance above the eligible amount can still leave you responsible for part of the bill.
