A monthly price for a weight-management service can mean several things: a consultation subscription, a medication estimate, or a package combining both. To compare your options, you need the complete cost and a benefit decision for the exact medicine.
This guide shows you what to ask, how to calculate your share and where to check BC PharmaCare and private-plan coverage. It applies to people researching Wegovy, Ozempic, Zepbound, Mounjaro and generic semaglutide.
The quick answer: Get an itemized pharmacy quote and confirm coverage using the exact DIN. Add assessment, follow-up and membership charges separately. There is no single BC price or coverage rule for all GLP-1 medicines.
What is included in the advertised price?
Before comparing clinics, identify what you are buying. Use the same supply period for both quotes.
| Cost item | What to ask |
|---|---|
| Initial assessment | Is this a one-time fee? Is it payable if no prescription is issued? |
| Medication | Which exact product, quantity and supply period does the quote cover? |
| Dispensing | Is the pharmacy fee included? |
| Membership | Is a subscription required to access follow-up or renewals? |
| Follow-up | How many visits are included, and what do additional visits cost? |
| Tests | Which tests are actually required, and who pays for them? |
| Delivery | Is shipping optional, and is it included? |
| Cancellation | What happens to fees, prescriptions and follow-up if I leave? |
Ask for the first-month total and the usual later-month total. A lower starting price can still produce a higher annual cost if follow-up or membership fees recur.
What BC's published cost estimates can tell you
The province's July 2025 medication table estimated annual maintenance-dose medication costs of C$5,600 for Wegovy and C$10,600 for Zepbound, excluding markup and professional fees. Dividing those historical estimates by 12 gives roughly C$467 and C$883 per month.
These are historical 2025 reference figures, not September 2026 retail quotes. They do not reflect every dose, newer generic product, discount, pharmacy or insurance arrangement. Use them to understand the scale of recurring costs, then obtain a current quote for your prescription.
Does BC PharmaCare cover these medicines?
The published evidence below describes specific decisions and pathways. It is not an individualized benefit determination.
| Product or pathway | What the source establishes | Practical next step |
|---|---|---|
| Semaglutide for diabetes | BC provides a Special Authority request process for eligible diabetes use. | Ask the prescriber and pharmacist to check current criteria and your exact product. |
| Wegovy | BC's December 18, 2025 decision records non-benefit status. | Check the current formulary for a newer decision; do not budget on assumed coverage. |
| Zepbound and Mounjaro | National authorization or reimbursement review alone is not proof of BC payment. | Search the exact DIN in the current formulary and check your private plan separately. |
| Generic semaglutide | A generic approval does not itself establish a benefit listing. | Check the DIN that the pharmacy will actually dispense. |
Sources: BC semaglutide Special Authority form, Wegovy decision, and the BC PharmaCare Formulary Search.
The formulary's “NB” notation means non-benefit. A national recommendation and a provincial formulary listing are separate steps. If the result is unclear, ask the pharmacy to explain what PharmaCare will recognize for your prescription.
Does Special Authority mean the medication is free?
No. Special Authority can establish coverage eligibility when criteria are met. Your PharmaCare plan, deductible, family maximum and other payment rules still matter. BC also says Special Authority coverage is not retroactive. Confirm the effective approval before relying on reimbursement for a purchase. See BC's pre-approval information.
Also separate the medical and payment decisions. A coverage approval does not determine clinical suitability, and a valid prescription does not guarantee the insurer will pay.
A short script for your insurer
You can copy this into a secure message to your benefits provider:
Please confirm coverage for the exact product and DIN on my prescription. Is prior authorization required? Does the prescribed use fall within my plan's coverage? Please explain my deductible, reimbursement percentage, eligible-price limit, annual maximum, any required generic substitution and the approval's effective and expiry dates.
Use the insurer's secure channel for personal and prescription information. Keep the response and reference number with your own records.
If the plan declines, ask for the reason. “Not covered” could mean a plan exclusion, an incomplete form, a missing approval, a product restriction or an exhausted limit. Each requires a different next conversation; none is fixed by choosing a clinic that advertises guaranteed approval.
A worked budgeting example
The following numbers are fictional and illustrate the calculation only.
Suppose an initial consultation costs C$100, the pharmacy quote is C$320 including dispensing, and an optional follow-up service costs C$40 each month. Assume the insurer confirms it will pay C$200 toward that month's pharmacy claim, with no other deductible in this example.
| Calculation | Amount |
|---|---|
| Pharmacy quote | C$320 |
| Less confirmed insurer payment | -C$200 |
| Your medication share | C$120 |
| Monthly follow-up | +C$40 |
| Initial consultation, first month only | +C$100 |
| First-month total | C$260 |
| Later-month total under these assumptions | C$160 |
For a year with one initial assessment and 12 unchanged monthly costs, the example becomes C$100 + (12 × C$160) = C$2,020. Real costs may change with quantity, treatment, plan limits or service fees. Replace every example figure with your confirmed quote.
Compare two services without mixing up the offers
Choose one supply period, such as 28 days, and record what each quote covers. Ask whether medication is included in a clinic subscription or billed by the pharmacy. Write down the cancellation rules and who remains responsible for follow-up.
A service with transparent fees, a clear clinician contact and a workable follow-up plan may be easier to use than one with an attractive introductory price and several unclear extras. The worksheet below gives you a place to compare those details.
What changes with generic semaglutide?
Use the exact generic product and DIN when asking for a quote. Existing coverage for a brand does not settle every substitution question. Our generic semaglutide update covers approvals, the Sevmia status check and the Dr. Reddy's recall notice.

Frequently asked questions
Is a clinic's membership fee the price of the medication?
Only if the offer explicitly says so. Ask for assessment, membership, follow-up and pharmacy charges as separate items.
Does diabetes coverage apply to weight management?
Do not assume that it does. Benefit criteria may depend on the prescribed use as well as the exact product.
Can I use the historical provincial estimate as today's price?
No. It is a dated reference with stated exclusions. Get a current pharmacy quote.
Can I compare cash and insured prices?
Yes. Ask the pharmacy to show the cash total, eligible insurer amount, insurer payment and your final share. Keep the product and supply period constant.
